1.  If you don't already have the free Adobe Reader installed on your computer,
download and install it:

2.  Open the Fed application, fill it in, and print it.

3.  Mail the application with your check to:

Annual Membership Dues
$20.00 per year for general members
$15.00 per year for students and seniors
For two-person households:  $30.00 for general members or $20.00 for students or seniors.

Your membership entitles you to discount prices on CDs and tapes at Boo Boo Records, reduced prices at selected jazz concerts, and our bimonthly newsletter.
